To understand that neither InclusivCare nor a provider can accept responsibility for the consequences of a patient refusing treatment or not complying with therapy instructions. To pay for services when they are provided. To inform InclusivCare staff about any changes in name, address, health history, telephone number, insurance coverage, financial status or health status. To provide InclusivCare with proof of individual or family income to establish discounted fees. To inform InclusivCare of your satisfaction with its care and services so that improvements to the health center can be made.
